Here I am providing you with information on how I use the different types of information that you provide and I will request your permission to do that in each case at your initial consultation:
- I take your contact details to remind you of your appointments, to send you aftercare information following an appointment if need be.
- I use information on your lifestyle to find out whether any factors may be affecting your health and well being and to discuss what you can do about it.
- I use information about your health to assess whether it is safe to go ahead with a treatment, whether written GP consent is required before proceding and/or whether there are any restrictions to treatment (e.g. a wound to avoid).
- I use information about your lifestyle and health to work out the most appropriate treatment plan for you (type, duration and frequency of treatments).
Note that I will not share your contact or personal details with third parties. The lawful basis for retaining data on lifestyle, medical history and past treatments is to provide the best possible treatment and advice in future. I will retain client records for a minimum of 7 years, and all details are kept secure.
